Which Form

ATF Form 7 (5310.12) covers dealer, pawnbroker, manufacturer and importer applications. Form 7CR (5310.16) is the collector application for a Type 03.

What Goes With It

An incomplete package is the most common cause of delay.

  • Completed application with the correct license type and fee
  • FD-258 fingerprint cards for every responsible person
  • Photograph for every responsible person
  • Responsible Person Questionnaire for each responsible person
  • A copy of the application sent to the CLEO at the time of filing

Consistency Checklist

Your legal entity name, trade name, premises address, mailing address and state records should match exactly across the application, your business filings and your local permits.
